Chronic Disease Management

Structured, long-term care for diabetes, hypertension, asthma and other chronic conditions — helping you live well, every day.

Living Well with a Long-Term Condition

Chronic diseases such as diabetes, hypertension and asthma are increasingly common in Uganda. With consistent monitoring, the right medication and good lifestyle support, most people with these conditions can lead full, active lives. At SAREC Medicare Centre, our physicians and nurses provide structured chronic-care reviews so that nothing important is missed between visits.

Hypertension (High Blood Pressure)

  • Accurate blood pressure measurement and confirmation of diagnosis
  • Baseline investigations — kidney function, electrolytes, lipid profile, urinalysis, ECG where indicated
  • Individualised medication regimens and adjustment over time
  • Lifestyle counselling on salt, diet, physical activity, alcohol and smoking
  • Screening for and management of complications (heart, kidney, eye)

Diabetes Mellitus

  • Screening with fasting blood sugar, random blood sugar and HbA1c
  • Diagnosis and classification (Type 1, Type 2, gestational diabetes)
  • Oral hypoglycaemic medications and insulin initiation where required
  • Self-monitoring education and review of home glucose logs
  • Annual screening for complications — kidney function, foot examination, eye review referral
  • Nutrition counselling tailored to the Ugandan diet

Asthma & Chronic Respiratory Disease

  • Diagnosis, severity assessment and personalised action plans
  • Inhaler technique training and review
  • Management of acute exacerbations and prevention of recurrence
  • Identification and avoidance of triggers

Cardiovascular & Other Conditions

  • Heart failure follow-up and medication review
  • Lipid disorders and cardiovascular risk reduction
  • Thyroid disorders — diagnosis and ongoing management
  • Chronic kidney disease monitoring and co-management with specialists
  • Gout, arthritis and other long-term musculoskeletal conditions

How Our Chronic-Care Programme Works

  • Scheduled reviews — regular appointments at intervals appropriate to your condition
  • Personal health record — we track your readings, medications and results over time
  • Medication refills from our in-house pharmacy for convenience and continuity
  • On-site laboratory for the tests required at each visit, with same-day results for most
  • Patient and family education — because understanding your condition is part of treating it
